Background: Primary lung schwannomas are extremely rare and mostly presents as a mediastinal tumor. There are only 62 patients with intrapulmonary or bronchial schwannomas reported by Ohtsuka et all, ranging from 5 to 83 years of age accounting a 0.2% of pulmonary neoplasm.
This is a case of an asymptomatic 71 years old male patient, without any history of systemic illness nor occupational or environmental hazards, who was referred to the pneumologist because of an incidental finding of a right parahilar soft tissue density on a CXR. Chest Xray described an oval shaped soft tissue density in the right parahilar region projecting between posterior aspect of the 5th and 6th ribs measuring 2.0cm x 2.5cm of indeterminate etiology. The patient was taken to a bronchoscopy procedure where a transbronchial biopsy was taken. Pathology report showed a positive S100 and CD56 markers, consistent with a diagnosis of a parenchymal schwannoma. PET CT scan was then ordered to asses the possibility of metastasis, and the study described a 2.1cm nodular mass in the posterior aspect of the right upper lung consistent with a known primary tumor without metastases. Our patient has been treated conservatively and followed periodically and is currently well.
This case illustrates a rare presentation of a pulmonary schwannoma found at parenchymal tissue rather than on mediastinum, where it usually presents. We add another case to the literature aside of the 62 patients previously reported, accounting for only 0.2% of pulmonary neoplasms.
