Purpose: This study aimed to evaluate the , especially, to assess the usefulness of decubitus radiographs in detecting foreign body aspiration. Method: We conducted a retrospective medical chart review of 32 children with suspected foreign bodies aspiration at a tertiary general hospital, between January 2012 and June 2017. They had chest AP or PA, chest down decubitus, chest CT and underwent bronchoscopy : 29 with chest AP or PA, 23 with chest down decubitus, 27 with chest CT, 29 with chest AP, PA or chest down decubitus, and 25 with bronchoscopy. Results: In 32 children with suspected foreign body aspiration, either unilateral emphysema or decreased radiodensity was suggestive of foreign body aspiration in chest radiographs. As a measure of detecting foreign body aspiration, the sensitivity, specificity, positive predictive value and negative predictive value of the radiologic findings were chest AP or PA group, 50%, 100%, 100% and 29.4%; chest down decubitus group, 57.9%, 50.0%, 84.6% and 20.0%; chest AP, PA or chest down decubitus group, 69.6%, 66.7%, 88.8% and 36.4%; chest CT group, 100.0%, 85.7%, 95.0% and 100.0%. The chest AP or PA, chest AP, PA or chest down decubitus and chest CT of detecting foreign aspiration were statistically significant. (P=0.039, P=0.030, and P=0.000) Conclusion: Standard chest radiography is taken as the initial imaging for patients with suspected foreign body aspiration. Decubitus chest radiographs seem to lack clinical benefit in evaluating young children with suspected foreign body aspiration. In the case of normal chest radiography and history of a witnessed choking episode combined with positive signs, the diagnosis of foreign body aspiration of the airway should be accomplished by using chest CT.
