A1951 - Association of Genetic Variants of NLRP4 Inflammasome with Smoking in Acute Exacerbation of Bronchial Asthma

Background: Cigarette smoking and deficiency of bronchial epithelial interferon may amplify airway inflammation which can evoke acute exacerbation of asthma. The primary aim of this study was to identify the association of smoking-related frequent exacerbation and NLRP4 polymorphisms in adult patients with bronchial asthma (BA). Methods: 1,454 patients with BA asthma were included in this study. Sixteen polymorphisms of NLRP4 were selected for genotype which was performed by single base extension. Results: All of 16 SNPs showed Hardy-Weinberg equilibrium. Asthmatic patients possessing rs1696718 minor allele (A) experienced more frequent exacerbation than rs1696718 common allele (G) homozygotes did (0.36 ± 0.04 vs 0.59 ± 0.06). Two or more acute exacerbation per year deeply related with SNP rs1696718 (Pcorr = 0.001 and 0.0005 in codominant and dominant model). In smoking subjects, the proportion of asthmatics possessing A allele of rs16986718 was significantly higher in frequently exacerbated group than other group (69.2 vs 47.3%, corrected P = 0.002 and 0.017 in codominant and dominant model, respectively). In A allele homozygotes, the number of exacerbation was significantly higher in smokers than that in non-smokers (1.28 ± 0.34 vs 0.28 ± 0.09, P = 0.011). The interaction between smoking amount and rs16986718 genotypes was significant (F = 1.628, P = 0.008) Conclusion: One of NLRP4 polymorphism, rs16986718, related with frequent acute exacerbation and served as a risk factor of acute exacerbation in adult patients with bronchial asthma. Additionally, a smoking has a different effect on the occurrence of acute exacerbation of asthma depending on the type of allele of rs16986718 .
