Introduction: Some COPD patients showed overlapped features of asthma. However, there are various definitions in asthma COPD overlap (ACO). The aim of this study is to compare prevalence of ACO according to specialist diagnosis, modified Spanish, Don Sin’s, and GOLD/GINA definition.Method: This was prospective multicenter observational study. Members of Korean Asthma Research Group prospectively surveyed various questions to COPD patients enrolled in Korea COPD subgroup study team cohort. Clinical characteristics of patients were also collected prospectively. Result: From 22 hospitals, 681 COPD patients were enrolled. Members of Korean Asthma Research Group diagnosed 29% of COPD patients as ACO. According to modified Spanish criteria, 13% of COPD patients were classified as ACO. However, only 4.7% of patients were classified according to Don Sin’s definition. It was extremely difficult to define ACO according to GOLD/GINA definition because of various patterns of checkbox. Conclusion: The prevalence of ACO was varied a lot according to various definition. GOLD/GINA definition was not practical in clinical practice.
