Rationale: Recent years, the proportion of older AIDS(Acquired immunode- -ficiency syndrome, AIDS)patients(≥50 years old) has increased rapidly , and PCP (Pneumocystis pneumonia, PCP) has become one of the most frequent AIDS-defining illnesses among them. There are limited information existing on older AIDS with PCP patients in China, thus we intended to investigate the clinical characteristics and treatment of these patients. Method: Patients with new AIDS diagnosis and ≥ 50 years old of age were defined as “older”. We collected and described the clinical symptoms, laboratory findings and treatment of 22 patients who are diagnosed as older AIDS with PCP patients. Results: There were 21 patients who were new HIV diagnosis, while only one patients found the infection 2 years ago. Only 5 patients admitted the sexual HIV acquisition, but the other patients refused to tell the doctors about the transmission pathway. As for symptoms, the frequency of fever, dyspnea and cough among these patients was 81.8%,77.3% and 68.2%. Although laboratory findings contributed little to the specific diagnosis of older AIDS with PCP, nearly all the patients had high Serum lactate dehydrogenase (LDH) levels over 220 u/l, and 72.2% of them showed high β-D-glucan levels (median, 147 pg/ml; range, 5-6920 pg/ml). All of the patients had a low level CD4 T-lymphocyte less than 200/mm3. Typical radiographic features of these patients are bilateral, symmetrical ground-glass opacities. High-resolution computed tomography (HRCT) test showed that the patients had ground-glass opacities(40.9%), while some of the others(45.5%) had reticulation, tree-in-bud appearance, cystic lesions, bronchiectasis lesion. All patients had some improvement on symptoms such as dyspnea and persistent fever after TMP-SMX treatment,.Half of them agree to receive standardized antiretroviral treatment in Specialized Hospital for infectious disease. Conclusions: Older AIDS with PCP patients in China have its own clinical characteristics and laboratory findings which is different from younger patients or other older AIDS with PCP patients in foreign country. More attention should be paid seriously on the possibility of AIDS with PCP among older individuals.
