A3910 - Spirometric Screening Behind the Latest World COPD and Asthma Days in Romania

Background: In the last years, on World COPD or Asthma Days, Romanian Society of Pneumology (SRP) organized 2 days awareness campaigns in Bucharest offering free spirometries for general public. The main reason was to spread awareness about asthma and COPD between Romanian population. Method and materials: we was used case finding approach, spirometry and specific asthma / COPD questionnaires symptoms, risk factors, previous lung function tests, history of exacerbations, previous respiratory therapy. Statistical analysis has been done using Microsoft Office Excel 2007. Results: A total of 692 persons (59.8% men and 40.2% women) with mean age=57.5±16 years were screened by spirometry. Lung function impairment had 22.1%(153) of them with mean FEV1=67±14%. Majority had a smoking history: active smokers (19.6% men + 8.7% women) versus former smokers (20.2% men + 8.7% women). From symptomatic ones 31.2%(216) had dyspnea, 27%(187) dry cough, 17%(118) productive cough, 21.9%(149) chest tightness and 24%(169) wheezing. Although 36.1%(250) were symptomatic with smoking history, 30.7%(213) of them don't ever performed lung function test. Moreover, 45.8%(317) were symptomatic with normal spirometry and 15.3%(106) cases with unknown respiratory condition were detected with abnormal spirometry. Even if 17.2%(119) persons had diagnosis of lung disorder (44-asthma, 26-COPD, 4-emphysema, 45-bronchitis) only 52 had treated their disease and 26 had performed spirometry in the last 6 months. Questionnaires analysis revealed the consequences of uncontrolled obstructive disease: 72.4%(501) had at least an exacerbation/year, 16% received antibiotic treatment, 4.7% presented to the emergency room, 2% required hospitalization. When it comes to whom is seeing patients on regularly basis 111 men and 90 women were seen by their GP, 59 men and 40 women by their pulmonologist. Conclusions:. Although addressability and accessibility to spirometry increases from year to year, pulmonologist was responsible for managing lung disease in only 14.3% cases. These obstructive pulmonary awareness campaigns were usefully for detecting new cases, but also for already diagnosed cases, who were not followed up properly, regarding treatment or assessing control.
