Eosinophilic pneumonia (EP) is characterized by massive pulmonary infiltration of eosinophils. Although serum periostin is a novel marker for eosinophil-dominant asthma, whether periostin is upregulated in the airway of asthmatics is controversial. In this study, we examined whether periostin concentrations are elevated in the bronchoalveolar lavage fluid (BALF) of patients with EP. BAL was performed in healthy volunteers and in patients with acute eosinophilic pneumonia (AEP), chronic eosinophilic pneumonia (CEP), and sarcoidosis, and periostin concentrations in BALF were measured. BALF concentrations of periostin significantly increased with pulmonary eosinophilia. Periostin concentrations in AEP or CEP patients were higher than those in healthy volunteers or sarcoidosis patients. Periostin concentration correlated with lymphocyte number and concentration of transforming growth factor-β1 in EP. These findings suggested that periostin may be involved in the pathogenesis of EP.
