A3636 - Longitudinal Patterns of Health Related Quality of Life in Patients with COPD

RATIONALE Health-Related Quality of life (HR-QoL) is considered to be an important predictor of major health outcomes in COPD patients. Since patients with COPD gradually experience degenerative breathing and physical functioning, understanding the process of dynamic change of health condition and quality of life along time flow, planning arbitration and management accordingly is very important. However, there are limited information on longitudinal changes of HR-QoL over time. This study is to identify the patterns of HR-QoL and sub-category in COPD patients over time in longitudinal data.
METHODS Data from 249 COPD patients enrolled in the 'Korean Obstructive Lung Disease' cohort form June 2005 to August 2012 were used. HR-QoL longitudinal data measured with the St. George's Respiratory Questionnaire were collected at the time of participation in the cohort and followed up every 12 months in 5 years. For data analysis, SPSS 20.0 and Stata 12.0 programs were used.
RESULTS The level of HR-QoL in the baseline of the subjects was found to be good at 34.2 ± 18.1 points out of 100 total points of SGRQ. The present study shows that HR-QoL slightly improved for 24 months, but continued to deteriorate over time. In the sub-domains analysis of HR-QoL, the symptoms domain which reflects the frequency and severity of respiratory symptoms over the past year, has continuously improved over time. However, activity domain which the reflects the level of daily life restricted by dyspnea showed improvement at the beginning, but it started to deteriorate from the first year, and impact domain which reflect social, emotional function and general disability degree slightly improved for 24 months then continued to deteriorate over time.
CONCLUSIONS The deterioration of HR-QoL is quite important to the individual patients with COPD. The subjects included in this study are registered in the cohort at medical institutions and are continuously administered medication administration and control, and it is judged that the level of respiratory-related symptoms is adjusted to a certain level. However, even though the symptoms are regulated, HR-QoL continues to deteriorate in the daily living ability, social and emotional difficulty the patient perceives. These results provided practical information about the time that health care providers should be actively involved in planning and providing interventions to improve overall HR-QoL.
