A1145 - High Degree of Fluoroquinolones Resistance Among Extrapulmonary Tuberculosis Patients at a Tertiary Care Center in North India

Rationale: Fluoroquinolones (FQs) drugs are one of the most promising antibiotic regimens for the treatment of drug-resistance TB. Drug susceptibility testing (DST) for FQs is not routinely assessed at baseline TB diagnosis. The limited information is available about FQs resistance among extrapulmonary tuberculosis (EPTB) cases.
Methods: Four hundred forty seven culture positive specimens were subjected to DST for first-line anti-tuberculous drugs (FLD) and second-line anti-tuberculous drugs (SLD) by using automated MGIT-960 liquid culture techniques, during the period of April 2016 to March 2017. Additionally, FQs drug susceptibility testing was also performed in FLDs sensitive strains.
Results: MTB was isolated in 447 specimens. Three hundred forty eight culture positive isolates were sensitive to FLDs, 64 were MDR-TB isolates and remaining 35 isolates were any drug resistance among FLDs. FQs monoresistance was observed in 4.3% (15/348) among FLDs sensitive strains and 21.9% (14/64) had FQs resistance among MDR-TB isolates.
Conclusion: The high degree of FQs resistance observed in EPTB specimens among drug sensitive and MDR-TB isolates is alarming. This study reflects the need to expand culture and DST for EPTB cases and include FQs within first-line DST in such settings. Also, FQs should be used rationally for treatment of other diseases.
