Objective The aim of this case-control study was to determine the relationship between serum cystatin C (sCysC) levels and lung function parameters in adult patients with chronic obstructive pulmonary disease (COPD). Methods Data were collected relating to all episodes of the inpatients who had lung function testing in the third affiliated hospital of Sun Yat-sen University between January and June of 2013. Variables of interest included age, gender, smoke, creatinine, glomerular filtration rate (GFR), MEF75, MEF25-75, serum cystatin C, one-year mortality and so on. The study was approved by the Institutional Review Board of the hospital. Results This study included 385 Chinese patients who were divided into COPD (n=127) and non-COPD (n=258) groups. Participants underwent lung function and laboratory testing, including measurement of sCysC levels. The first logistic regression analysis identified the predictors of COPD included age, smoke, neutrophil/lymphocyte ratio (NLR), sCysC, blood urea nitrogen, 25% of the forced vital capacity (MEF25), 50% of the forced vital capacity (MEF50), 75% of the forced vital capacity (MEF75), 25-75% of the forced vital capacity (MEF25-75). In the second multivariable analysis that smoke (P 0.003, OR 0.371, 95%Cl 0.191-0.717), sCysC(P 0.035, OR 1.833, 95%Cl 1.045-3.217), MEF75(P 0.003, OR 0.048, 95%Cl 0.006-0.367) and MEF25-75(P 0.014, OR 0.053, 95%Cl 0.005-0.555) were still associated with COPD. There was no significant difference about creatinine, GFR and one-year mortality between two groups. Conclusions low MEF75 and MEF25-75, high sCysC levels were predictors of adult patients with chronic obstructive pulmonary disease.
