We report the successful extraction of a bronchial foreign body (BFB) which had strongly penetrated into bronchial mucosa using microwave coagulation therapy (MCT). An 82-year-old man, who was visiting another hospital with a diagnosis of asthma for several years, was hospitalized at other hospital due to pneumonia diagnosis with a BFB in the left main bronchus on chest radiograph and computed tomography. He was a favorite crab’s food and had habitually ate it with shells when he was a fisherman. He had unconsciously aspirated and spent as an asthma patient for a long time. Flexible bronchoscopic examination showed a BFB strongly penetrated into bronchial mucosa in the left main bronchus. We later recognized it as a crab's scissors leg tendon. The BFB was removed with a flexible bronchoscopy using MCT to prevent bleeding and release the infiltration. This is the first report to remove a crab's scissors leg tendon by using MCT.
