Around 170,000 subjects have the two-days medical checkup every year in Japan even though their costs are very expensive. Lung age have been used for assessing some changes in smoker for the few decades. Most of our work has focused on difference between real age and lung age. But in the one-day medical checkup had a curiously point that the difference between real age and lung age of ex-smoker was younger than never-smoker. This time, we focused on the two-days medical checkup. Therefore we considered the following, we checked 2408 subjects who were hospitalizing St. Luke’s International Hospital for two-days medical checkup since 2004 to 2012. Only 2060 subjects have no obstructive lung disease. We checked their smoking history. At last nine years we have studied data from 1024 non-smokers, 742 ex-smokers and 294 smokers for lung age. All subjects had spirometry and physical examinations. We calculated their lung age at hospitalized two-days medical checkup. The difference between real age and lung age were significantly lower in female non-smokers compared to female ex-smokers 2.58±13.00 vs 8.33±13.60 yrs. The difference between real age and lung age for female smokers 15.50±15.40 were higher than ex-smokers. Male non-smokers are 4.61±13.71; ex-smokers 10.93±13.40; smokers 13.54±13.98. There were signify difference in each male group. This study suggests that the influence of smoking were thought to be large and for smokers than ex-smokers. In future study we have to explain this difference between the one-day medical checkup and two-days.
