In this session we will provide a general overview of social determinants of health and discuss why they are important. We will review the agenda of national organizations, including Healthy People 2020, the Center for Disease Control, and the Center for Medicare and Medicaid Services, in addressing social determinants. In addition, we will review the existing evidence of patient outcomes related to social determinants of health. Attendees will learn how to incorporate screening for social determinants of health in their patient population.
